Declaration of Physical Suitability and Consent

The undersigned declares under his own responsibility:

To hold a valid sports medical certificate for underwater activities.
Of not being aware of pre-existing medical conditions (heart, lung problems, epilepsy, etc.) that may preclude the practice of diving.
Of not having taken alcohol or drugs that could alter the state of consciousness in the 12 hours preceding the activity.
To be aware of the inherent risks of underwater activity, including but not limited to: decompression sickness, traumatic air embolism, nitrogen narcosis, and marine-related risks.

Equipment Rental

In case of rental of the equipment provided by the center, the undersigned:

It accepts the equipment in the state it is in, declaring that it is forward-looking and found to be in good working order.
It undertakes to use the equipment diligently and to return it in the same condition.
It assumes financial responsibility for any damage, loss or theft of the rented equipment.

Disclaimer

The undersigned, by signing this form, exempts and relieves the Free Divers Italy diving center, its instructors, guides and collaborators, from all civil and criminal liability for:

Accidents, personal damage or death that may occur during diving or transport by boat.
Damage or loss of personal items brought on board or left on the premises of the centre.
Incidents resulting from negligent conduct by the participant or failure to comply with instructions provided by staff.

Regulatory Awareness (Law No. 70 of May 7, 2026)

The undersigned declares, under his or her personal responsibility, that he or she is aware of the provisions introduced by Law No. 70 of 7 May 2026 regarding underwater activities (containing regulations for diving centers, diving schools, clubs and private individuals). It also declares that it has reviewed the text of the aforementioned law, displayed visibly and legibly within the office premises, and is committed to full compliance with the requirements, limits, and safety requirements it establishes for carrying out the activity.

Privacy Policy (GDPR)

The undersigned consents to the processing of his/her personal data pursuant to EU Regulation 2016/679 for the purposes closely related to the provision of the service and legal obligations.
